If your loved one is in immediate danger, call 911. If the situation is not an emergency, document what you see, take photos when appropriate, write down dates and staff names, save messages from the facility, and request records. You may also report concerns to the facility administrator, Stanislaus County Adult Protective Services, the Long-Term Care Ombudsman, or California Community Care Licensing.
